After Acceptance
Once you have been accepted to PCA, the following documents will need to be completed and returned prior to enrollment:
- Enrollment contract and tuition deposit ($250/student)
- Health Questionnaire
- PCAP Parent/Student Contract
- Parent/Student Contract, Grades 1-8
- Records Release Form
- Computer Contract, Grades 3-12
- FACTS Form, if applicable
- Bus Registration Form and deposit, if applicable
- Parent Partnership Program Survey (for new families)
